Pedigree Form Latest Revision: April 21, 2006.
Complete this form along with the Registration Application when applying for Spotted Draft registration.
Indexing Requirements Form Latest Revision: April 21, 2006.
Beginning June 1, 2005, you must complete and submit this form when Index registering a horse. Must be completed IN ADDITION to the Registration Application.
Breeding Certificate Latest Revision: April 21, 2006.
A Breeding Certificate is signed by the owners (at time of service) of the Sire & Dam of the horse being registered, and must be submitted with the Registration Application. Not required for Index Registration.
DNA Analysis Latest Revision: February 9, 2005.
All Stallions used to produce Spotted Draft foals are required to have a DNA Parentage test on file. Submit this form with hair sample as instructed on form.
Stallion Breeding Report Latest Revision: April 21, 2006.
All NASDHA™ Registered Stallions must have a Stallion Breeding Report submitted in each breeding year for subsequent foals to be eligible for registration.
Transfer Form Latest Revision: April 21, 2006.
Sellers must complete and submit the Transfer Form when selling a registered Spotted Draft horse.
Correction Certificate Latest Revision: April 21. 2006.
Complete and submit a Correction Certificate if a horse has been gelded, deceased, original papers have been lost/damaged or correcting color, brands, or markings.
Membership Application Latest Revision: April 21, 2006.
Become a new member or renew your existing NASDHA™ Membership.
Prefix Application Latest Revision: April 21, 2006.
Members may register a Prefix with the Office to use when naming their foals.
